
PY23 HEDR: The results are in
Health Equity Data Reporting (HEDR) gives ACO REACH participants the opportunity to earn back up to 0.2% of their ACO Benchmark by collecting beneficiary-reported demographic and Social Determinants of Health (SDoH) data. With Mabel’s help, 9 ACO REACHs achieved more than 90% of their PY23 HEDR adjustment — representing nearly $2M in incremental profit across our partner network.

PY23 Learnings & Observations
Across our partner ACOs, the organizations that performed best treated HEDR as a year-round operational priority rather than a single submission event — building survey collection into existing patient touchpoints, training providers and staff on why the data matters, and following up systematically with beneficiaries who hadn’t yet responded.
PY24 SDoH: What’s your strategy?
PY24 raises the bar. CMS has heightened expectations around SDoH data collection, and ACOs that haven’t built a deliberate strategy risk leaving meaningful benchmark dollars on the table. Some of the biggest PY24 SDoH challenges we’re hearing from ACOs include:
- Reaching beneficiaries who don’t have a recent in-person visit scheduled
- Collecting sensitive SDoH data in a way that feels respectful and low-friction
- Coordinating survey outreach across multiple provider groups and EHRs
- Keeping response data organized and submission-ready ahead of CMS deadlines
