Health Equity Data Reporting (HEDR) gives ACO REACH participants the opportunity to earn back up to 0.2% of their ACO Benchmark by collecting beneficiary-reported demographic and Social Determinants of Health (SDoH) data. With Mabel’s help, 9 ACO REACHs achieved more than 90% of their PY23 HEDR adjustment — representing nearly $2M in incremental profit across our partner network.

PY24 SDoH: What’s your strategy?

PY24 raises the bar. CMS has heightened expectations around SDoH data collection, and ACOs that haven’t built a deliberate strategy risk leaving meaningful benchmark dollars on the table. Some of the biggest PY24 SDoH challenges we’re hearing from ACOs include:

  • Reaching beneficiaries who don’t have a recent in-person visit scheduled
  • Collecting sensitive SDoH data in a way that feels respectful and low-friction
  • Coordinating survey outreach across multiple provider groups and EHRs
  • Keeping response data organized and submission-ready ahead of CMS deadlines
