GUIDE is a voluntary, risk-free, 8-year model running from July 1, 2024, through June 30, 2032. It introduces a new payment structure for participants who deliver a comprehensive care delivery framework — including comprehensive, person-centered assessments and care plans; 24/7 access to a support line; care navigation; and respite services to support caregivers.
As ACOs and health systems gear up to participate, Mabel will expand the capabilities of our voluntary alignment and beneficiary engagement platform to support GUIDE-specific workflows, including:
- Voluntary alignment
- Diagnosis attestation
- Beneficiary and caregiver assessments
- Sociodemographic and HRSN data
